Semaglutide is a synthetic GLP-1 receptor agonist with 94% sequence homology to native GLP-1, modified with a C18 fatty diacid chain attached via a linker to lysine at position 26. This acylation enables albumin binding in plasma, extending the half-life to approximately 7 days and enabling once-weekly dosing. Semaglutide has received regulatory approval in multiple forms (Ozempic for type 2 diabetes; Wegovy for obesity; Rybelsus oral formulation) and represents the most extensively studied GLP-1 receptor agonist in both preclinical and clinical research.

GLP-1 Receptor Pharmacology

GLP-1R agonism produces a coordinated metabolic response: glucose-dependent insulin secretion from pancreatic beta cells (preserving hypoglycaemia safety), glucagon suppression from alpha cells, delayed gastric emptying reducing postprandial glucose excursions, and central hypothalamic appetite suppression via GLP-1R expression in the arcuate nucleus and brainstem.
